Use IRS Form 1040X, which can be obtained from this site www.irs.gov

You will need to state the reason for filing an amended return and show corrected tax information, as well as your original refund or balance due amount.

Do not attach your original return to the amended return.If you are expecting a refund, you must file the amendment within three years after the date you filed your original return, provided it was filed timely.
